New Client Information

All required Intake Paperwork, including informed Consents Must be completed prior to your first session. During your initial phone call with your treating therapist, they will provide instructions on how to access the Electronic Health Record through Office Ally. All required treatment documents will be completed on-line.

Please note: for our families that are separated or divorced our office requires treatment consent for minor children from both (all) legal guardians.
